Safety
Policy and Program
D&M
Concrete Floor Co., Inc and its affiliates believe that safety
and production go hand in hand. An efficient, productive job
is also a safe job.
The
safety policy of D&M Concrete Floor Co, Inc and its affiliates
encompasses the following principles:
- Furnish employment
and a place of employment which are free from recognized
hazards that are likely to cause death or physical harm
to their employees.
- Abide by
and enforce the safety and health regulations promulgated
by the federal state and local governments.
- Instruct
their employees to recognize and correct unsafe conditions,
use the proper procedures for handling hazardous substances
and to practice recognized safe work methods.
- Guide the
corporation’s operations by a safety program.
Safety Program
To effect the safety
policy, the following program shall be implemented. The basic
feature of the program is safety responsibility. Each supervisor
is responsible for the employees under his/ her direct supervision.
The program is not to be limited to its specific provisions
which serve as only a guide.
Article A
Management
Responsibilities
Management shall
coordinate the Safety Program. This shall include, but not
be limited to:
- Provide the
leadership and determination to carry out the safety policy.
- Enforce the
safety policy and discharge any employee who willfully
disregards or violates the safety policy.
- Require all
subcontractors to comply with the safety program and all
local, state, and federal requirements.
- Encourage
other prime contractors working on the site to cooperate
with the safety program.
- Survey each
project periodically to evaluate its participation in
the safety program. Document the resultant findings to
project supervisors.
- Administer
the safety incentive award program (detailed elsewhere).
- Administer
subcontractors’ certificate of insurance compliance
program.
- Maintain
accident records and statistics
- for each
project (monthly for its life);
- for total
operations (monthly and yearly).
- Keep D&M
Concrete Floor Co., Inc. and its affiliates abreast of
changes in federal , state and local safety and health
regulations.
- Keep D &M
Concrete Floor Co., Inc. and its affiliates abreast of
new developments in the field of construction safety and
health.
- Participate
in the Associated General Contractors of America local
safety committee meetings and functions.
- Confer with
the insurance carrier regarding the handling of claims
and loss control services provided to the various projects.
- Conduct periodic
company wide safety meetings to discuss the problems and
progress of the safety program.

Directive
This notice is
to advise and admonish any and all parties in any way involved
with the operations of D&M Concrete Floor Co., Inc. and
its affiliates, that the management of D&M Concrete Floor
Co., Inc, and its affiliates regards its safety policies as
one of the top priorities of its management functions, and
expects and will insist that all supervisors, workman and
subcontractors give safety and the preventions of accidents
the same top priority attention.

Responsibilities of Job Project Manager/Superintendent
The job Project Manager/ Superintendent is responsible for
placing this program into effect, enforcing the safety policy,
and for the total safety function. This includes but is not
limited to:
- Be familiar
with all rules and regulations pertaining to construction
safety and health standards and ascertain compliance with
same. The more common standards are outlined in Appendix
B.
- Instruct
foreman on the principles of the safety policy. Train
field supervisors to recognize construction hazards, accident
causing conditions and improper/unsafe work methods and
practices, and how to alleviate/eliminate the same.
- Confer with
each foreman on a daily basis if need be to ascertain
that the following are being considered and acted upon
prior to and during actual construction operations:
- Planning:
Layout to provide the proper space and arrangement of
materials, product, and refuse.
- Storage:
Piles to be stable and without readily visible projections;
movement of men and materials not to be blocked or restricted.
- Working
Area: To be sufficient; tools, controls, and materials
to be within proper reach.
- Guards
and Safeguards: To be in place and proper for the application;
safeguards include all barriers, guardrails, handrails,
hole covers, devices, and measures to control exposure.
- Machinery:
To be used, installed and maintained so that failures
will not produce hazards.
- Equipment:
To be frequently checked to see that it meets current
standards and that wear has not changed its effectiveness;
equipment to be matched to the tasks to be performed.
- Methods:
To use equipment designed for the specific purpose.
- Manpower:
To be adequate and proper for the tasks to be performed.
- Work Pace:
To be reasonable for the tasks at hand.
- Training:
Instruct and train employees in proper methods, skills,
and safe practices; job hazards and safe practices to
be understood by employees.
- Direct
Supervision: Follow up instructions by observing employees’
work habits to ensure safe work practices are being
followed.
- Enforcement
of Work Rules: To be established and known to all employees.
- Safe Work
Practices: To be enforced for every job.
- Mismatched
to Job: Physical abilities to match job requirements.
- Lighting
or Ventilation: To be adequate for a safe and healthful
work environment, per state and federal standards.
- Fire and
Panic: Procedures to be established for the adequate
control of each; fire equipment to be adequate and serviceable
for immediate use.
- Appoint a
qualified first aid certificate holder for the jobsite.
Larger jobs may require more than one first aid certificate
holder.
- Make available
to all employees applicable safety materials, first aid
supplies and equipment, sanitary facilities and drinking
water. See Appendix A.
- Make available
to all employees the necessary personal protective equipment
and ascertain that said equipment is properly worn and
maintained in a serviceable and sanitary condition. See
Appendix A
- Make available
the necessary auxiliary protective equipment such as ventilation,
insulated mats, hazardous gas meters, etc., and ascertain
that said equipment is maintained in a serviceable and
sanitary condition. See Appendix A.
- Review each
incident with all the foreman. Ascertain that each foreman
thoroughly investigates each accident experienced by his
own crew to determine the cause(s) and then fully documents
the Supervisor’s Report of Accident Investigation
report form. Follow up each accident to determine that
corrective action has been taken to reduce/ eliminate
the probability of a similar accident recurring. Records
of accidents shall be maintained and posted as required
by OSHA. These records indicate trends, areas needing
more work and general program progress. See Appendix A.
- Conduct weekly
safety meetings on all jobs. On larger jobs, assign qualified
company personnel to conduct the safety meetings with
a number of groups, such that all employees are included.
The Project Manager/ Superintendent should arrange to
attend these meetings whenever possible. Consult with
the group leaders to determine that their weekly safety
meeting subject matter is germane to the work at hand.
- Conduct periodic
meeting with all the subcontractors to advise them that
they are required to adhere to the safety program as well
as all the rules and regulations promulgated by OSHA.
The wearing of required personal protective equipment,
the maintenance of guards and safeguards, housekeeping,
proper material storage and placement and safe work methods
are among the items to be stressed.
- Require subcontractors
to attend the weekly safety meeting.
- Periodically
review all of the above subparts, and survey the job for
the compliance with all of same. Document to Company management
all discrepancies noted and the action that has been taken
to correct such discrepancies.
- Appoint a
knowledgeable safety person who will take the company
check list of safety items and make a safety inspection
of the whole job on a daily basis. All items in compliance
will be checked, all items not in compliance will be marked
with notations for required action. The Project Manager
/ Superintendent shall take immediate steps to rectify
the items not in compliance. Copies of these check lists
shall be sent to the main office for management attention
on a regular basis.
- Set up procedures
for the investigation of all accidents to determine the
causes and see that corrective action is taken.

Responsibilities
of the Job Foreman
The job foreman
is responsible for safety at the work crew level. This responsibility
includes but is not limited to :
- Instruct
the work crew on the principles of the safety policy.
- Ascertain
that the provisions of subparts No. 1,3,5,6,7,8, and 11
of Article C are all carried out at the work level.
- Correct all
recognized unsafe conditions immediately by directing
his crew or specific members of his crew to perform those
acts required to alleviate said condition(s).
- Correct all
recognized unsafe and improper work methods and practices
as observed while performing their normal supervisory
function by on the spot, or aside in private as circumstances
dictate, consultation with the crew members who are noted
to be engaging in unsafe methods and practices.
- Insure that
all injuries are cared for immediately and reported promptly
to the Superintendent.
- Conduct weekly
safety meetings with his own crew ( or in combination
with other foremen and their crews as the job becomes
less labor –intense). The subject matter of the
weekly safety meeting shall deal with (a) the hazards
associated with the work being performed or to be performed;
(b) improper work methods and practices noted by the supervisor
during the week prior to the meeting and; (c) any accidents
which occurred during the week prior to the meeting. If
none of the preceding are germane to any particular safety
meeting, then the subject matter may delve into principles
of general safety. Each weekly safety meeting is to be
documented on the Weekly Safety Meetings form. This form
is to be filled out completely.
Article E
Responsibilities of the Workmen / Women
Workmen have the
responsibility of complying with the Occupational Safety and
Health Ace of 1970, Public Law # 91-596 (OSHA), and its updated
revisions. In addition workmen will:
- Work in accordance
with the safety policy given to them.
- Refrain from
any and all unsafe practices.
- Report to
their foreman any unsafe conditions or practices that
they have observed.
- Use and maintain
properly all the safety and or protective equipment that
the company provides them.
- Immediately
report to their foreman any and all accidents/ incidents
involving personal injury no matter how slight or seemingly
minor.
- Immediately
report to their foreman any and all accidents/ incidents
involving damage to property/ equipment/ tools no matter
how slight or seemingly minor.
- Submit to
their foreman a dated and signed statement by a qualified
doctor to the effect that the employee may return to work
after being absent from work due to any and all work/non-work
related injuries/illnesses when the services of a doctor
are required to treat said injuries/illnesses.
- Report for
work in a condition not effected by any illegal drugs
or alcohol and refrain from the use of any such drugs
or alcohol on the jobsite. Failure to comply with the
above can result in dismissal or discharge.

Article
F
Responsibilities
of Subcontractors
Subcontractors
are required by contract to comply with this safety program
as well as all the rules and regulations promulgated by OSHA
while working on our project(s) and are responsible to the
limit of safety laws for their own acts of commission or omission.
Article
G
Protective head
gear (hard hats) shall be worn by all persons at all times
while on our construction projects with the following exceptions:
- While in the
office trailer/ office.
- While in
a vehicle or material handling equipment fitted with a
full protective cab.
- By finish
trades persons working inside a structure where there
are no hazards or conditions which would require the wearing
of said head gear to alleviate or minimize said hazards.
Conclusion
D&M Concrete
Floor Co., Inc. and its affiliates are obligated to provide
a safe and healthy work place for their employees, subcontractors,
the employees of their clients while engaged on the job premises
and to the general public who come in contact with D&M
Concrete Floor Co., Inc’s. construction operations.
This safety policy and program are intended to discharge that
obligation with care and completeness.
